Home First NHS CCG Consultation

Home First - your future care at home and in the community (consultation)

Please see information below from NHS Mid Essex CCG

Within mid Essex, there are three areas of current care that we would like to change in a bid to offer local people the best outcomes for their health.

  1. In 2018, the CCG plans to introduce a Home First service that will aim to help more people rehabilitate and recover at home after a spell in hospital
  2. We want to maximise the use of two state-of-the-art theatres at Braintree Community Hospital for Orthopaedic surgery
  3. At the same time, we would like to change the way beds on Courtauld ward at Braintree Community Hospital are used to help patients to recover after surgery.

More information about these changes can be found in the attached booklet and on our website: http://bit.ly/2y9XYWC

We would encourage you to forward this information on to any colleagues or groups who may be interested.

We are particularly keen to hear from anyone with experience of receiving care in a community hospital, those waiting on CHC assessments and their carers/family and friends.

How to share views

We want to find out what you think about our plans for Home First and Braintree Community Hospital. There are a variety of ways to get involved in discussions, find out more and share views.
